- However, If you don’t have this code, then you can also click on “If you do not have a 26-digit code printed on your receipt, click here.”. After that, you will need to provide various details like store number, visit date, time, and amount spent.
- Once the survey is started, you will have to answer various questions about your recent visit to McDonald’s.
- Here you can share your feedback related to your visit, quality of food, service, cleanliness, and staff’s behavior.

I tried to take your survey, but, I could not get down to put the numbers in. So, I shall do it this way. I stopped at your McDonald’s store at 320 colony blvd., the villages, fl. 32162. I ordered a fish sandwich, with only mustard, a small fries, & a small drink. My bill came to $7.04. When my food was brought to me, the sandwich had cheese and I don’t know what else on it. I just saw the cheese and took it up to the counter.
The lady who waited on me wrote “mustard only” on the order. I am allergic to cheese and do not eat anything with cheese or has cheese in it. When the new sandwich was brought to me, the cook must have been angry, as my sandwich was “thrown” UPSIDE DOWN in the box and the the bottom of the sandwich was not even on the fish correctly – it was half on & half off. By the time I got my sandwich, my fries were maybe “luke” warm. Needless to say, I am not happy with that particular restaurant, and I don’t believe I want to ever go there again. The McDonald’s store on 466 in the villages near wedgewood lane is a much nicer store and my food has always been correct. I have never complained before, but, this time I felt like someone in authority should know about how a person’s “re-done” sandwich is handled. Thank You
I went to McDonald’s three times in NSB, Fl. The people there at this store are warm and friendly, especially Robby. I can’t get into your fields so I am writing them in. This is the first survey I have filled out and I do think it is a sham. I take my Downs Syndrome grand daughter there all the time so I will see if I get a response.
I tried to fill out survey but the receipt is not legible. I got a breakfast sandwich, a sausage muffin . It was not edible. The muffin was very stale and the sandwich tasted salty. Awful.
The service was good . Grace is always friendly but this is the worst food I have ever gotten from this store.
